ABI Intensive Case Coordination and System Navigation

ABI Intensive Case Coordination (ICC) and Occupational Therapy (OT) Support

Traverse Independence offers community-based supports for individuals with brain injuries through our ABI Intensive Case Coordination (ICC) and Occupational Therapy (OT) services.

Intensive Case Coordination (ICC)

Our ICC, Dan Holisek, collaborates with various service providers to ensure continuity and coordination of services. Dan works in partnership with community organizations, primary care, mental health, addictions, housing providers, and other service providers to address the needs of this complex population. Many clients requiring ICC support face significant and complex barriers. Dan is skilled in system navigation from an acquired brain injury perspective, providing personalized, flexible, and individualized care plans, typically short-term. Outcomes include skill building, new coping strategies, goal achievement, and more.

Occupational Therapy (OT)

Kendra Leighton, our experienced OT, focuses on assessing and supporting individuals with ABI, neurodivergence, persistent pain, and mental health challenges. Kendra offers community-based support with a brain injury lens, including daily routines, tools, memory aids, environmental adjustments, and other interventions that positively impact individuals with brain injuries.
