ABI Mainstream Outreach

Traverse Independence offers mainstream outreach services: our community facilitator works with you and your family in your home. 

In some cases, ongoing weekly support is needed, in others, just monthly check-ins. The community facilitator works with your community-based rehab team to apply goals and strategies to daily living in the following areas of independent living:

  • Scheduling appointments and transportation
  • Budgeting and money management
  • Menu planning, shopping, cooking
  • Medical appointment attendance and support
  • Implementing rehabilitation programming
  • Household tasks
  • Transportation training
  • Other supports as required

Land Acknowledgement

We acknowledge that we are located on the Haldimand Tract, traditional and unceded territory of the Anishnaabe, Haudenosaunee and Neutral Peoples. It encompasses 980,000 acres of land that was given to the Six Nations in 1784, 10 kilometers on each side of the Grand River.
