Ideally, the gains made during the in-house phase (transitional living) must be transferred to the community. This requires consistent support from familiar and trusted staff that allows you to transition to the place where you will live. To make that a reality, Traverse Independence offers outreach services: our community facilitator works with you and your family in your home. The community facilitator is there to support you to establish the routines in your new place of residence.
In some cases ongoing weekly support is needed on a long term basis. The community facilitator works with your community based rehab team to apply goals and strategies to daily living in the following areas of independent living:
