The referral process starts with your information request. But as you are unique, we will contact you to find out more about your needs and set up a tour or information session at your home or at our sites.

Eligibility criteria
GENERAL PROGRAM ELIGIBILITY
The applicant must:
- Be at least 16 years of age.
- Be a Canadian citizen or landed immigrant and have a valid Ontario Health Card.
- Have a permanent physical disability or brain injury.
- Provide verification of disability through medical documentation from a valid regulated health practitioner.
- Have any medical issues stabilized with ongoing clinical supervision.
- Not be dangerous to themselves or others or have behaviours that will be disruptive to effective service provision.
- Not have ongoing psychiatric or substance use issues that will be disruptive to effective service provision.
- Have a free will commitment to rehabilitation and/or participation in programs.
- Be able to pay for any costs associated with program participation.
SPECIFIC PROGRAM ELIGIBILITY
ABI Transitional LIVING
The applicant must:
- Be safe should they leave the transitional living setting both on a daily or permanent basis.
- Demonstrate the desire and/or potential (with or without support) to participate in the program.
- Be safe when left alone or demonstrate this capacity for the future.
- Have a suitable plan for housing upon discharge.
- Require 4 hours or less of personal support per day, and 4 hours or less a week of community facilitator support. The applicant must show the potential to decrease the staffing support throughout the duration of the program.
ABI Group Home
The applicant must require twenty-four hour supervision along with personal support.
ABI Outreach Program
The applicant must:
- Participate in the development of their goals and work consistently on the completion of those goals.
- Reside in the Region of Waterloo or County of Wellington.
- Reside in their own personal home including a retirement home. Applicants living in LTC facilities are not eligible.
ABI Day Program- Opportunity Centre
The applicant must:
- Have have a level of independence that ensures his/her safety should he/she choose to leave the Opportunity Centre at any time during program hours.
- Not reside in Long Term Care.
- Reside in the Region of Waterloo or County of Wellington.
ABI Day Program- Beginnings Program
The applicant must:
- Require personal support services.
- Not reside in Long Term Care.
- Reside in the Region of Waterloo or County of Wellington.
