ABI Intensive Case Coordination and System Navigation

ABI Intensive Case Coordination and Occupational Therapy support at Traverse Independence can provide community-based case coordination, system navigation and occupational therapy supports for individuals who have an ABI.  This can be assistance with connecting to additional resources and appropriate community supports, including ABI resources across Ontario

Intensive Case Coordination works to provide linkages and collaborations with other service providers to ensure continuity and coordination of services. Case coordination works in partnership with community organizations, primary care, mental health, addictions, housing providers, and all other service providers that assist in addressing the needs of this complex population.

Many of the clients who require the support of intensive case coordination have significant and complex barriers. The Intensive Case Coordinator is skilled in system navigation from an acquired brain injury perspective. Each client is provided with a personalized, flexible, and individualized care plan, which most typically is short term in nature.

Please welcome our new ABI Specialized Intensive Case Coordinator and System Navigator

Dan Holisek studied Kinesiology at the University of Waterloo, where he obtained his Bachelor of Science (BSc) Honours degree. Towards the end of his studies, his interest moved towards neuroscience and where he received the opportunity to work with Traverse Independence (TI). This partnership was to assist with developing a combination of assessments that are verified for use in the field of acquired brain injury - the ultimate goal was to be able to collect information on individuals at intake (baseline) and then again at discharge, through the programs offered at TI to have a quantitative measure of progress. This project led to Dan continuing to work with TI since 2015 to the present. In that time, Dan has worked in many different capacities at various locations within TI to support a wide range of individuals all presenting with very complex and differing challenges. Dan hopes to continue supporting individuals by reducing barriers to service and ensuring that a holistic approach is always explored and utilized.

Welcome to our new Occupational Therapist

We are fortunate to welcome a new Occupational Therapist to our team. Please join us in welcoming Kendra Leighton to Traverse Independence!

Kendra has been an occupational therapist since 2015 after having her sights on becoming an OT since high school.  She completed her undergraduate studies in Kinesiology at the University of Waterloo and minored in psychology followed by her Masters in Occupational Therapy at Dalhousie University. Since then, she has gained an array of experience working in adult home care, motor vehicle accidents, private pediatrics and adult populations.  Her training and experience have significantly focused on assessing and supporting individuals with ABI, neurodivergence, persistent pain, and mental health challenges. She is passionate about social justice and equity and promoting quality of life.  In her spare time, she loves to run, cook (and eat of course!), do yoga, and spend time with family and friends.

Kendra, we are so pleased to have you join our team, and we all look forward to working with you.

Please visit our Apply for Services page to access our application forms for clients with acquired brain injuries.
