ABI in the Streets is a low barrier program, supporting those who have triggered a positive HELPS screener, are considered homeless or precariously housed. While brain injuries challenge many people with mental health issues and substance use, it is apparent that those who are homeless are impacted at an even higher rate due to their circumstances.
Please note that, if the client has safe permanent housing, can be reliably contacted, is interested in working on specific goals, and can attend appointments/meetings they would be more appropriate for the ABI Mainstream Outreach Program.
The Region of Waterloo Community Services Council has recommended strategies to end homelessness for all people in our region. Traverse experience and data from the ABI in the Streets program show that there is a very high prevalence of brain injury in this population and we are pleased that the presentation makes clear reference to this fact.
* It is important to note that the HELPS tool is a screener and NOT a diagnostic tool, but Traverse will accept clients with a positive screener for the mainstream Outreach, drop-in centre and Intensive Case Coordination.
Launching Ocean eReferral Network
Traverse is excited to launch The Ocean eReferral Network on Weds. February 15th.
The Ocean eReferral Network is changing how healthcare referrals are sent and managed, replacing traditional fax-and-paper systems, while keeping patients informed every step of the way.
The Ocean eReferral Network is an integrated Electronic Medical Records system and is a cloud-based technology for healthcare referrals. The Network includes a map-based, searchable directory of healthcare providers with wait times, intelligent referral forms, end-to-end reporting, and automated status alerts for patients and providers.
Patients are kept informed about the status of their referrals and get better, faster access to care in their community.
Healthcare Providers can easily send structured referrals right from the patient’s chart, view wait time information, and reduce delays from incomplete requisitions and faxes.
Specialists receive more appropriate referrals, with more complete information, while eliminating time consuming, paper-based workflows.
