We are pleased to announce that the Waterloo Wellington LHIN has agreed to convert the ABI nurse practitioner to an ABI intensive case coordinator. Many of the clients who require the support of intensive case coordination have significant and complex barriers. This position will re-engage with the client, assessing their needs, engaging in ongoing planning and implementation of plans. This model of support will have a worker who is skilled in system navigation from an acquired brain injury perspective. Each client will have a personalized, flexible, and individualized approach and care plan, which most typically will be short term in nature.
